Where an incident has happened, it triggers the Duty of Candour, staff will report this to the PM/GP/Owner. They are responsible for ensuring the Duty of Candour procedure as activated. The PM will inform HIS and a significant event analysis (SEA) will be created to allow feedback and improvements to be made.
